From sagging skin to the appearance of fine lines, we all notice the visible changes in our skin as we age. To assist our clients in ageing beautifully and gracefully, we chat to Dr Wade Merchant, our new Medical Aesthetic Doctor
at Legs eleven Green Point.

Dr Wade Merchant is a qualified Medical Doctor with a special interest in aesthetic and anti-aging medicine. He holds a certificate in aesthetic medicine which he gained through the American Academy of Aesthetic Medicine. He is also specially trained by Allergan to inject Botox and Fillers. In addition, he holds a certificate in Aesthetic Mesotherapy – accredited by the World Anti-Aging Mesotherapy Society. He is also trained and certified to perform the Silhouette Soft Thread Lift, Medical Micro-needling (Dermapen) as well as Medical Grade Peels. Dr Merchant is registered as an Independent Practitioner with the HPCSA. He is also a member of the South African Medical Association and The Aesthetic and Anti-Aging Medicine Society of South Africa.

Our faces age as a result of several anatomical changes that occur as we get older. Not everyone ages at the same rate – all of these changes are influenced by our genetics, diet, alcohol consumption, smoking, environmental pollution exposure as well as sun exposure. If we control and limit these factors, the aging process can be slowed down. That said, facial aging is one of life's cruel truths – it happens to everyone. Luckily thanks to recent scientific advances, this process can be diagnosed and treated – much in the same way that any medical problem is treated. This evolving field is known as Aesthetic Medicine.


Sagging is one of the main issues that concerns clients these days. Our faces sag because of loss of fat in our cheeks. The result is a line below your eye (the tear trough), a line between your nose and corner of the mouth (the nasolabial fold) and a line between the corner of your mouth and jawline(the marionette line and jowls).cheek volume loss

Rather than just injecting and chasing lines, the current most natural approach to facial rejuvenation is volume restoration. The easiest and safest way to do this is by injecting non-permanent hyaluronic acid fillers into the depleted fat pads of the cheeks. Once this has been done, tightening and lifting procedures can be introduced. In addition to these in-office procedures, it is absolutely essential that every patient has an at home care plan that enhances skin quality and texture.

Our faces also sink inwards and downwards as a result of bone loss – over time it gets reabsorbed. An example is the cheek bone, which can be easily treated using a thick gel like filler to provide deep support to the overlying tissue. Besides fat and bone loss, our skin also becomes less effective at producing collagen – this starts as early as thirty years of age. Collagen is the molecule that allows the skin to be tight and firm – so less of it means sagging as well as poor quality skin texture.

Medical Aesthetic treatments and their role in treating facial aging:

1. Refining:

Botulinum Toxin Type A – this anti-wrinkle injection gently relaxes the muscle that it is injected into. As the muscle is weaker, it forms less wrinkles when it contracts. Thus, this injectable is used to treat wrinkles that are caused by muscular movements (dynamic wrinkles): Frown, Crow's feet (eye wrinkles) and the forehead. The treatment takes a few days to start working and lasts approximately 3 to 4 months. Fine lines are reduced and eyes appear more youthful.

2. Volume Replacement:

Hyaluronic Acid Fillers – they are injected into areas of the face that are volume depleted. The aim is to rejuvenate and refresh the area. Treatment areas include the cheeks, under eye area, mouth, lips, chin as well as the temples. Depending on the type of filler used, the results can last anywhere from 6 to 18 months. The inverse triangle shows a lifted more youthful appearance to the face, post treatment!

3. Lifting:

The most effective current method of non-surgical facial lifting is with the use of re-absorbable threads. With threads, our focus is mainly in treating the lower face - the jowls as well as the nasolabial folds. Silhouette Soft Threads last 18 months – but results may last longer as they are collagen stimulators as well. The Silhouette Soft Thread Lift can only be performed by medical doctors as it is a specialised procedure that requires adequate knowledge of facial anatomy as well as a large degree of skill. There are many types of threads on the market – not all threads will give you the same results as this brand. Jowls are lifted and the jawline appears smoother, and more youthful.

4. Resurface:

One TCA pigmentation BAChemical peels are used to create a controlled burn in the skin. The affected skin then peels off and a new revitalised skin is revealed beneath. TCA (trichloroacetic acid) peels are medium depth peels that are used to treat fine line, wrinkles as well as discolorations. The downtime involves some peeling and redness and skin is usually back to normal within a week.

5. Collagen Stimulation:

  1. Dermapen Micro-needling
    Thousands of needle punctures stimulate the skin to recruit fibroblasts (specialised skin cells) – they then help to produce collagen. Needling can help to treat acne scarring, fine lines and wrinkles, as well as discolorations (as the pigment cells known as melanocytes are also stimulated).

  2. Restylane Skin Boosters
    This is a very fluid-like hyaluronic acid based filler that is used to hydrate skin, fill fine lines and wrinkles as well as to stimulate collagen production. This product can be used on anyone, young or mature – and is great for anyone as a first time aesthetic treatment.

  3. Tri-fractional Radio-Frequency
    Here a controlled electrical impulse creates micro-thermal trauma within the skin. The skin then attempts to “heal” itself – and an increase in collagen production occurs. In much the same way as needling, it can be used to treat acne scarring, fine lines and wrinkles.

6. Revitalise:

Mesotherapy – this involves a series micro-injections that are composed of vitamins (such as vitamin C), anti-oxidants (like glutathione) as well as retinol and hyaluronic acid. The aim is to deliver active ingredients directly into the skin so that they can get to work immediately. The aim is to revitalise and hydrate skin – and once again the treatment can be performed on anyone.
